Mortgage Checklist

ALL LOAN APPLICATIONS (including refinance)

[ ] Copy of your Purchase Contract, signed by all buyers and sellers (for purchase transactions)

[ ] Original pay stub(s) covering the most current 30-day period for each applicant (for example, if you are paid twice a month, you would need to bring two pay stubs); continue to save your pay stubs until you close your loan

[ ] Original W-2 forms for the last two years for each applicant

[ ] Original bank statements for the past two to three months for all checking and savings accounts (all pages) with explanation and proof of deposits not from payroll; continue to save your bank statements until you close your loan

[ ] Name, address and phone number of landlord for the last 12 months if you are currently renting or have rented in the last 12 months

[ ] Original 401(k) and IRA statements; include all pages (if applicable)

[ ] Original investment and account statements for the past three months (include all pages); mutual fund accounts are an example (if applicable)

[ ] Last 12 months of canceled mortgage payment/rent checks (if applicable)

[ ] Photocopy of valid driver’s license or acceptable photo ID

[ ] Photocopy of Social Security card

[ ] Social Security Awards Letter when social security income is being used; if Social Security is direct deposit, only a copy of the borrower’s bank statement showing award is required; Page 1 of federal income tax return required when grossing up Social Security income

[ ] Disability Awards Letter when disability income is being used

[ ] Retirement Income Awards letter when retirement income is being used; if retirement is direct deposit, only a copy of the borrower’s bank statement showing award is required

[ ] Pension income, if received through an annuity, must have a letter documenting continuance of income for three years

[ ] Homeowners Insurance Binder with coverage equal to loan amount; deductible cannot exceed 1% of loan amount or $1,000, whichever is less

[ ] Warranty Deed (if property is being refinanced)

[ ] Copy of original title insurance and closing paperwork (if property is being refinanced)

[ ] Copy of current mortgage statement (if property is being refinanced)

[ ] If you have graduated from high school or college within the last two years, enclose a copy of your diploma or transcripts

[ ] If during the past two years you have a gap in your employment of 30 days or more, include a letter explaining the reason

[ ] If you have closed on your present home, provide a copy of your signed HUD-1 Settlement Statement showing the amount of proceeds (If the sale is not yet complete, please provide a copy of the Purchase Offer)

[ ] If you are being relocated by your employer, provide a copy of your company’s relocation policy

[ ] If you are receiving a gift for part of your down payment, you will be given a gift letter at time of application. In some cases, the borrower may need to provide copy of donor’s bank statement and a copy of the transfer or deposit of the gift

[ ] If you are a first-time home buyer and eligible for your local finance authority’s assistance program, provide a complete copy of last year’s federal income tax return (Please sign in blue ink)

[ ] If you have declared bankruptcy in the last seven years, provide a copy of Petition Decree, Schedule of Creditors, and Discharge, as well as a letter explaining the reason why you filed bankruptcy

[ ] If you have rental property, provide a copy of current lease and copies of last two years of signed federal income tax returns (Please sign in blue ink)

[ ] Credit report reviewed with borrower to make sure all credit is disclosed

[ ] Resident card and work authorization card for permanent and non-permanent resident aliens


IF YOU ARE APPLYING FOR A VA LOAN

[ ] VA Certificate of Eligibility

[ ] Form DD-214 or, for in-service veterans, Statement of Service if discharged within the last two years

[ ] Most recent Leave and Earnings Statement (in-service veterans only)

[ ] Name and address of child care provider (if applicable)

[ ] Addendum to initial application fully completed, signed and dated


IF YOU ARE APPLYING FOR AN FHA LOAN

[ ] Addendum to Purchase Contract

[ ] Addendum to initial application fully completed, signed and dated

[ ] Provide FHA worksheet on refinance transactions


IF YOU ARE SELF-EMPLOYED, HAVE COMMISSION INCOME, ARE A TRUCK DRIVER, MINISTER OR HAVE OUTSIDE SALES

[ ] Copies of your last two years of personal and business federal signed income tax returns including all K-1’s (Please sign in blue ink.)

[ ] Year-to-date Profit and Loss Statement and Balance Sheet (self-employed only)


IF YOU ARE DIVORCED

[ ] Complete signed copy of all divorce decrees and separation agreements, including any stipulations or modifications

[ ] Proof of receipt of child support or alimony payments for the last 12 months (if you are using this income to qualify for your mortgage loan or if you are required to pay child support) per investor requirements
